At Foot Centre Group, we are dedicated to providing comprehensive podiatry services to support your overall health, particularly when managing chronic conditions. If you have a chronic medical condition, your GP may recommend a Chronic Disease Management Plan (CDM) and a Team Care Arrangement (TCA) to help coordinate your care.
What is a Chronic Disease Management Plan?
A Chronic Disease Management Plan (CDM) is designed for individuals who have a chronic or long-term medical condition lasting six months or more. This plan allows your GP to work with allied health professionals, such as our podiatrists, to ensure you receive the best possible care for managing your condition.
What is a Team Care Arrangement?
A Team Care Arrangement (TCA) is part of the CDM and involves a coordinated approach where your GP, our podiatrist, and other health professionals (like physiotherapists or dietitians) work together as a team to provide you with the most effective care. This approach ensures that all aspects of your health are addressed by specialists in their respective fields.
Understanding Medicare Rebates
While Foot Centre Group is a private clinic, patients eligible for a CDM and TCA can receive Medicare rebates for up to five podiatry consultations per calendar year.
How does it work?
Payment: As a private clinic, you will need to pay the full cost of your consultation upfront.
Medicare Rebate: After the consultation, you can claim a partial rebate from Medicare. The rebate amount will depend on the consultation type and your Medicare plan, but it helps reduce the overall out-of-pocket cost.
